To request a copy of your medical records please complete an Authorization Form for Release of Confidential Health Information. Please make sure to include a mailing address in your request as Discovery Support Services is unable to send your records via fax. When completed, submit the release form to the Diamond Headache Clinic, Attn: Medical Records.

Most medical records requests will be processed within 10 business days from the date the request was received. If it has been more than 10 business days please call our office at (312) 388-6390 xt.402 to check the status of your request.
