Requesting your Medical records…
The Diamond Headache Clinic now uses SDS (Smart Document Solutions) as our medical records copy service. SDS is the world’s largest health document processor and has been processing medical record requests since 1976.
To request a copy of your medical records please complete an Authorization Form for Release of Confidential Health Information. Please make sure to include a mailing address in your request as SDS is unable to send your records via fax. When completed, submit the release form to the Diamond Headache Clinic, Attn: Medical Records.

Most medical records requests will be processed within 10 business days from the date the request was received. If it has been more than 10 business days please call our office at (773) 388-6315 to check status on your request.
